L7-lO-7. Form of affidavit to be printed on envelope - General, spe'

cial or municiPal elections'

Each absentee ballot shall be accompanied by an envelope upon which shall

lrinted an affidavit. This affrdavit which shall be used in general, special

municipal elections shall be substantially as follows:

ts) The official ballots for any election to which this chapter pertains shall be

in ihe same form as the official regular ballots for the election, except that they

Iiall have printed thereon the words, "Official Absentee Ballot" and except that
Iiere shall also be printed thereon one of the affidavits hereinafter prescribed.

&) Such ballots shall be sworn to before a notary public or other officer
.ulhorized to acknowledge oaths, except in the following cases:- (1) A person confined on aecount of physical disability to a hospital, nursing

home, rest home or sanitarium may in person execute the affidavit on the

ballot before the authority in charge of the hospital, nursing home, rest home

or sanitarium where he or she is confined or before a notary public.
(2) Any applicant for an absentee ballot who is a member of the armed

forces of the United States, as enumerated in seetion 1?'1G3, and is on active

duty in such armed forces or the wife of any such member of the armed forces,
who will be away from his or her residence on election day, may make the
affidatit on the ballot before any officer authorized to administer oaths or
before the commanding officer or other authority in charge of the unit of the

national guard, reseryes or other eomponent of the armed forees. (Acts 19?5,

The form of the affidavit which shall be printed on absentee ballots used in
general, special or municipal elections shall be substantially as follows:

"State of Alabama

"Before me, the undersigned authority, personally appeared

who is known (or made known) to me and who, being first duly sworrl, deposes

and says: I am a bona fide resident and qualified elector of. . . .

precinct or district in . . . .

County, Alabama or that I was a bona fide resident and qualified elector
of . . . . . . . precinct or district until my removal therefrom on the
day of . . . ., 19. . . ., a date not more than thirty days before the election
day, and but for such removal I still satisfy the Alabama registration
requirements. I have not heretofore voted in the election to be held
on ., 19....,
and I am entitled to vote therein; but, I will be away fnim the county
of my residence (or former residence) or unable to go to the polls on such day.
Therefore I have marked the foregoing absentee ballot and hereby declare
the same to be my ballot in said election.
