To begin the processing of this appeal, your first responsibility 
is to complete the transcript order form provided to you by the 
District Court Clerk. When completed, this fulfills your 
obligation under FRAP 10(b) to order the necessary portion(s) of 
the court reporter's transcript. This office will then 
coordinate with the court reporter the time for filing the 
transcript. If extensions to this time are allowed we will 
inform you. Failure to timely order the transcript can result in 
dismissal of this appeal as set out in Taylor vs. S & D 
Enterprises, Ltd., et al., 5 Cir., 1979, 601 F.2d 175. 

Information regarding the briefing of this appeal will be 
furnished to all parties at a later date. 

All counsel desiring to appear in this case must sign and return 
the attached appearance form within fifteen (15) days from this  
date as required by Circuit Rule 46. Failure to do so indicates 
that you do not desire to appear and your name will be removed 
from our docket in this case. Also, we cannot release official 
records on appeal unless an appearance has been entered.
